Carry out social work tasks to a satisfactory professional standard in accordance with the policies of the Social Services Committee and the instructions of the Service Unit Head. Comply with and meet the standards set out by the policies and procedures jointly agreed between Social Services and Dorset Healthcare University NHS Foundation Trust.
Account to the Service Manager for the quality of work and provide information on the nature of the work.
Ensure that support plan reviews are carried out in a timely and person-centred way, evidencing the need for continuing support.
Undertake comprehensive assessments, contribute to risk assessments and risk management plans for adults.
Perform care coordinator duties for designated caseloads in accordance with Dorset Healthcare policies and procedures.
Conduct carers assessments and collaborate with families.
Engage in preventative work by providing information and advice to adults and their carers to promote health, effective service use, and personal responsibility.
Support individuals to look after themselves and their families, promoting choice, control, and the use of personal budgets and direct payments where appropriate.
Undertake best interest assessments, considering the Mental Capacity Act requirements.
Assist adults in understanding the support and care options available from the Council, ensuring resource use aligns with person-centred outcomes.
Act as an Approved Mental Health Professional or undertake training to do so, including making hospital admissions under the Mental Health Act.
Serve as a Safeguarding Adult Practitioner or undertake relevant training.
Train as a Deprivation of Liberties Assessor.
Act as Care Manager as required, delivering services in line with statutory responsibilities and eligibility criteria.
Maintain accurate records for statistical and informational purposes, complying with quality standards and using core computer systems.
Participate in team and official meetings, including risk, child protection, safeguarding meetings, and others.
Contribute to out-of-office hours standby and statutory functions with proper preparation and authorization.
Engage in training and manage a caseload of a specialized nature, providing recovery-focused interventions.
Provide consultancy services within the team or service unit.
Take part in development roles within the team or unit.
Prepare for supervision and appraisal, demonstrating continuous professional development.
Prepare reports for court and attend legal proceedings as needed.
Undertake liaison roles, such as dual diagnosis link worker.
Perform other duties as required, consistent with the level of the post.
Comply with all relevant policies, statutory requirements, and legislation including equality, health and safety, and data protection laws.
Maintain registration with SWE where required and undertake related statutory tasks in accordance with legislation and competence frameworks.
