Overview
We are a passionate Integrated Community Network Team dedicated to enabling individuals of all ages to live life on their own terms. Our work spans complex mental and physical health challenges, partnering with health colleagues to generate holistic assessments and achieve positive outcomes in Bridgend.
Responsibilities
* Collaborate with Social Workers, District Nurses, Occupational Therapists, Physiotherapists, Dietitians, Speech and Language Therapists, Pharmacy, and Local Community Coordinators to assess, support and meet outcomes for adults affected by long-term, complex conditions and their carers.
* Work alongside GP surgeries and third sector partners to deliver community-based care and reduce unnecessary hospital and care home admissions.
* Support family members and carers as part of Strengths-Based, Outcome‑Focused practice, facilitating wider inclusion in the community.
* Protect children, young people or adults at risk as part of council responsibilities.
Qualifications
* Ability to greet customers through the medium of Welsh.
* Enhanced with Adults Barred list criminal records check (DBS).
* Desire to work in community-based, therapeutic settings and a commitment to continuous development.
