Advanced Clinical Practitioner - GP Practice
Goyt Valley Medical Practice (GVMP) are looking for a full-time or part time/job share ACP to join our supportive team of GPs, ACP, Pharmacist, Practice Nurses, HCAs and Admin Staff.
GVMP as a practice embraced the role of ANPs in general practice over 20 years ago and has been proud of the strong team of ANPs and then ACPs that we have employed since then and the role that they play within the team. The successful applicant(s) will join a highly motivated and supportive team, filling a vacancy left by a highly valued team member.
Main duties of the job
GVMP is a total triage practice, using AccuRX, with the allocating role being undertaken by all GPs and ACPs on a rota basis, alongside 1 on-call per week.
The ACP will work as part of this team working to deliver high quality clinical careboth in the practice and on home visits. The role will involve triage, assessment,diagnosis and treatment planning for patients across the age range with supportfrom the rest of the team.
About us
We are a strong and cohesive practice making full use of the advantages of multi-disciplinary working and allowing clinicians to make full use of the skills that they have developed. We are also a training practice taking both F1s and F2s on a regular basis. Any successful applicant will have the opportunity in being involved in the training and development of these Junior Doctors.
We run regular weekly clinical meetings as well as more frequent catch-ups as well as more formal training sessions.
Job responsibilities
Main Clinical Responsibilities
* Provide the first point ofcontact for clinical queries from practice telephonists/receptionists on a rota basis.
* Be responsible for telephone andonline triaging on a rota basis.
* Undertake urgent/same day appointmentsas well as routine appointments.
* Undertake home visits.
* Assess, diagnose, plan,implement and evaluate treatment/interventions and care for patients presentingwith an undifferentiated diagnosis
* Clinically examine and assesspatient needs from a physiological and psychological perspective, and planclinical care accordingly
* Assess, diagnosis, plan,implement and evaluate interventions/treatments for patients with complexneeds
* Diagnose and manage both acuteand chronic conditions, integrating both drug and non-drug-based treatmentmethods into a management plan
* Initiate and interpret testsand investigations as appropriate.
* Prescribe (where appropriatelyaccredited) and review medication for therapeutic effectiveness, appropriate topatient needs and in accordance with evidence-based practice and national andpractice protocols, and within scope of practice
* Work with patients in order tosupport compliance with and adherence to prescribed treatments
* Qualified Prescribers: Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions
* Prioritise health problems andintervene appropriately to assist the patient in complex, urgent or emergencysituations, including initiation of effective emergency care
* Assess, identify and referpatients presenting with mental health
* Support patients to adopthealth promotion strategies that promote healthy lifestyles, and applyprinciples of self-care
* Record accurate data in thepatient records to an agreed standard.
* Collaborate with other membersof the Primary Health Care Team including doctors, nurses and AHPs, acceptingreferrals and referring to them for specialist care.
* Direct referral to and liaisonwith consultants and other professional agencies.
* Ensure a high standard ofclinical practice is maintained by ensuring care is based on current bestpractice, embarking upon further training if required and continuing to updateclinical knowledge.
QualityRequirements
* Recognise and work within owncompetence and professional code of conduct as defined by their statutoryregulator
* Produce accurate,contemporaneous and complete records of patient consultation, consistent withlegislation, policies and procedures
* Be aware of data protection(GDPR) and confidentiality issues particularly within a GP surgery.
* Use technology and appropriatesoftware as an aid to management in planning, implementation and monitoring ofcare, presenting and communicating information
* Review and process data usingaccurate SNOMED codes in order to ensure easy and accurate informationretrieval for monitoring and audit processes.
* Prioritise, organise andmanage own workload in a manner that maintains and promotes quality
* Deliver care according to NICEguidelines and evidence-based care
* Assess effectiveness of caredelivery through self and peer review, benchmarking and formal evaluation
* Keep up to date with currentevidence-based practice.
* Participate in research andutilise the audit cycle as a means of evaluating the quality of the work ofself and the team, implementing improvements where required
* In partnership with otherclinical teams, collaborate on improving the quality of health care respondingto local and national policies and initiatives as appropriate
* Support and participate inshared learning across the practice and wider organisation
* Assist in the development andimplementation of policies and procedures
* Understand and apply legalissues that support the identification of vulnerable and abused children andadults, and be aware of statutory child/vulnerable patients health proceduresand local guidance.
* Participate in annualprofessional development review. Personal development will be encouraged andsupported by the practice. It is the individuals responsibility to remain upto date with recent developments. Training requirements will be monitored by annualappraisal and will be in accordance with practice requirements.

Person Specification
Qualifications
* - Nurse, paramedic, pharmacist or other registered healthcare professional, registered with the professional council as appropriate, with no restrictions on practice or pending Fitness To Practice concerns
* - Completion of recognised Advanced Practice qualification at Level 7, demonstrating as a minimum advanced clinical assessment, clinical reasoning, diagnostics
* - Qualified and competent to see children
* - Independent Prescriber able to work independently without direct supervision
