Overview
Our nurses are committed to teamwork, have good communication and decision making skills, and consistently work collaboratively with other professionals within HCT and beyond. We aim to provide a high standard of patient care using an evidence-based framework.
Successful applicants will be responsible for assessment, planning, implementation, and evaluation of individualised nursing care to patients living in the community. They will support the Nursing Sisters of the team to manage the caseload and support junior staff. A commitment to health promotion, teaching and training is essential to optimise patients’ ability to self-manage their health needs and engage in maintaining their wellbeing.
Responsibilities
* Assess, plan, implement and evaluate individualised nursing care for community-dwelling patients.
* Support the Nursing Sisters to manage caseloads and supervise junior staff.
* Promote health education and training to enable patients to self-manage and maintain wellbeing.
Qualifications and Training
* RGN (Registered General Nurse).
* Car driver and holder of a current UK driving licence (unless you have a disability as defined by the Equality Act 2010).
* Evidence of professional development.
Experience and Knowledge
* Understanding of current health legislation.
* Understanding of the HCT organisation.
* Experience in managing a ward or team (desirable).
Desirable Criteria
* Caseload management.
* Experience working with other agencies.
Skills and Abilities
* Verbal and written communication skills.
* Good record keeping.
* Interpersonal skills, clinical audit skills, supervisory skills, and IT skills.
