Main duties of the job
As a member of themulti-disciplinary team, you will be expected to provide prescribing advice andsupport to practice clinicians, staff, and patients. This includes prescriptionand medication queries, medicines policies and procedures, QOF and localincentive schemes, audit, and medicines safety.
Clinical work at the individual practices will includemedication reviews, minor illness consultations and reviews post discharge.
You will have individual supervisionfrom a GP and PCN Prescribing Lead, and be partof an established team of Pharmacists and Pharmacy Technicians. Job responsibilities
The post holder will be an experienced pharmacist, who acts withintheir professional boundaries, to develop, manage and mentor a team ofpharmacists and pharmacy technicians to include regular clinical supervision, 1 to 1support and development. They will be anon-medical prescriber and have completed the Centre for Pharmacy PostgraduateEducation *CPPE* National Pathways and supervisor courses.
They willwork within theirclinical competencies as part of a multi-disciplinaryteam to provide senior expertise and clinical leadership in medicines optimisation andquality improvement across the PCN. This willinclude structured medication reviews with direct patient contact and may include contributing to:
* Enhancing Healthin Care Homes
* Management of long-termconditions
* Management of medicines on transfer of care
* Reviewing systems forsafer prescribing
* Contributing to achievement of National and Local Contracts (e.g. IIF,QOF, CEIF, QI)
* Undertaking clinical audit
* Contributing to repeat prescription authorisations and reauthorisation
* Actioning acute prescription requests
* Addressing both the public health and social care needs of patients

Person Specification
Experience
* Minimum of 3 years post-qualification experience
* Demonstrates the ability to communicate complex and sensitive information in an understandable form to a variety of audiences
* In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare and strategies for improving prescribing in primary care, particularly for long term conditions
* Demonstrate good IT skills, with experience of spreadsheets, databases and reporting functionality
* Able to influence/motivate/persuade others with an empathic / reassuring manner, where there may be significant barriers
* Demonstrates a clear understanding of best practice and legal requirements relating to prescribing and medicines storage
* Ability to manage a team and provide individual clinical support and mentorship
* Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
* Able to identify and resolve risk management issues according to policy/protocol
* Experience of partnership/collaborative working and of building
* relationships across a variety of organisations
* Experience of investigating complaints and medication incidents
* Demonstrates leadership experience and previous experience of supervising and mentoring more junior staff
Qualifications
* Holds, or working towards, an independent prescribing qualification
* Successful completion of the CPPE
* Evidence of CPD and study beyond registration in line with requirements for revalidation and demonstrating commitment to lifelong learning
* Specialist knowledge acquired through post-graduate diploma level or equivalent training/experience of working at an advanced level
* Successful completion of the Clinical supervisor training for CPPE national training pathways
