This role is ideal for a nurse passionate about delivering high-quality care to older adults with complex needs.**Key Responsibilities**Supporting consultant-led geriatric & frailty clinicsCompleting comprehensive patient assessments, including cognitive and mobility reviewsManaging patients with common geriatric conditions such as:Frailty & falls riskDementia & cognitive impairmentMultiple long-term conditions (multimorbidity)Polypharmacy concernsMobility issues & deconditioningContinence issuesSupporting coordination of tests, referrals & community follow-upProviding patient and family/carer educationSupporting telephone/virtual follow-up clinicsAccurate record-keeping within NHS electronic systemsWorking closely with Consultants, Frailty Teams, Physiotherapists, OTs, HCAs & Social Care teams**Requirements****Registered Nurse (NMC PIN required)**Minimum **1 year experience** in Geriatrics, Frailty, General Medicine, or Community NursingStrong understanding of elderly care pathwaysExcellent communication, compassion & patienceAbility to manage complex cases and vulnerable patientsConfident working both independently and as part of an MDTYou must live in the UK and have the right to work in the UK**What We Offer**Competitive NHS-aligned pay ratesFlexible outpatient shiftsWeekly payrollFast-track complianceOngoing support from a dedicated consultant**Location:** Mid West Wales (NHS Outpatient Clinics) **Contract:** Part time Flexible Weekend Work **Employer:** *SJK Healthcare* #J-18808-Ljbffr
