Reconnect Practitioner
* Location: Leeds/Wakefield
* Job Type: Monday - Friday - Full time and part time available
* Pay rate : £25.78 per hour
We are seeking an experienced Mental Health Nurse to assess and support individuals attending the local Magistrates Court, focusing on health and social needs through an all-vulnerability model.
Day-to-Day of the Role:
* Conduct comprehensive assessments focusing on the health and social needs of service users.
* Support the Liaison and Diversion service aimed at reducing reoffending by addressing health and social needs.
* Provide support and advice to service users through the Magistrates Court process.
* Use assessment information to prepare and provide reports to the courts to aid in decision-making.
* Communicate necessary adjustments and provide options to court staff.
* Undertake assessments of vulnerable individuals in police or court custody and share relevant information with the Police, Court, and Crown Prosecution Service.
* Develop and deliver workshops to enhance team knowledge on court processes and stakeholder engagement.
* Support junior team members and contribute to audits and service development.
Required Skills & Qualifications:
* Experience working with vulnerable service users, particularly in mental health, substance misuse, or criminal justice.
* Degree or equivalent experience relevant to the post.
* Strong understanding of multi-disciplinary and multi-agency working.
* Excellent communication skills and a compassionate, non-judgmental approach.
* Knowledge of the Mental Health Act 1983 (amended 2007), PACE (Police and Criminal Evidence Act 1984), and other relevant legislation.
* Proficient IT skills and IT literacy.
* Proactive problem-solving skills and flexibility in working arrangements.
