Role Overview
The Support Worker plays a crucial role within Changing Lives, providing information, advice and guidance through referral and direct individual support. The aim is to empower the people we support to live independently and work towards overcoming the interconnected problems that lead them into accessing our services.
Key Activities / Responsibilities
* Working both individually and as part of a team to build productive relationships with individuals accessing the Changing Lives service.
* Providing support, listening, guiding and signposting individuals to enable them to receive the required support to manage their physical environment.
* Building relationships on a 1:1 basis, as well as within group environments, to provide key support and enhance personal wellbeing.
* Carrying out regular building health and safety inspections, identifying risk factors, responding accordingly, and performing basic repairs where required.
* Ensuring that the standards of the building are maintained by following designated tasks, maintaining a high level of cleanliness, and meeting all legal and statutory requirements.
* Empowering individuals to improve life skills through delivery of workshops/activity sessions and/or signposting to external agencies that can support in this area.
* Ensuring that full written reports, documentation and internal systems are updated in line with Changing Lives' policies and procedures, maintaining confidentiality and adhering to GDPR.
Project Specific Activities
* Supporting residents to gain independent living skills such as cooking, shopping and using the washing machine.
* Supporting residents to appointments within the community such as attending probation, drug and alcohol services, and doctors' appointments.
* Building robust move‑on plans and supporting residents to independent living.
* Leading activities to engage residents in meaningful use of time.
* Signposting and making referrals to other agencies such as domestic abuse services, drug and alcohol services, mental health services etc.
* Raising safeguarding referrals, MARACS, MAPPA and attending multi‑agency meetings.
* Carrying out allocated daily cleaning tasks.

We support people who are experiencing homelessness, or at risk of becoming homeless, as well as providing specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women and young people. Accommodation to us doesn't just mean a roof over your head and shelter from the elements; it needs to be a welcoming and flourishing environment, offering opportunities for all those we support to feel comfortable and develop.
About the Project Open House Plus
Open House Plus, located in Doncaster, is a 26‑room homeless hostel. The project provides supported accommodation for individuals with a number of complex needs. As a Support Worker at Open House Plus, you will have the opportunity to join a passionate team dedicated to empowering those we support. You will receive training on all aspects of the role and take part in case management reviews with the whole team.
