If so, an exciting opportunity has arisen for an experienced healthcare assistant to join our multidisciplinary team working across the Glossopdale Community.
Main duties of the job
The post holder will be a skilled and experienced Healthcare Assistant responsible for supporting the care of patients with a variety of acute and long-term health conditions. As part of the new Team Up initiative, they will provide care within their professional scope through community-based home visits. Their role will include delivering and assisting clinical staff with treatment, preventative care, health promotion, and patient education.
Key responsibilities include performing baseline clinical observations such as pulse oximetry, blood pressure, phlebotomy, BMI, and temperature checks as directed, and accurately recording results. The post holder will also contribute to chronic disease management and deliver opportunistic health promotion where appropriate. They may be involved in completing basic care plans.
A travel allowance will be provided for undertaking home visits.
About us
Glossop Primary Care Network (PCN)serves a population of approximately 33,000 patients across four practices: Manor House Surgery, Cottage Lane Surgery, Lambgates Health Centre, and Simmondley Medical Practice.
This role will contribute to the delivery of the Team Up initiative, a new collaborative care model being implemented across Glossop. Team Up brings together primary care, social services, and the voluntary sector to provide integrated care to patients in their own homes aiming to reduce hospital admissions wherever possible.

Key Responsibilities
* Deliver basic clinical care and health promotion interventions as delegated by registered staff, including basic observations, phlebotomy, nutrition.
* Monitor vital signs and report changes in condition to the supervising clinician.
* Assist in the implementation of care plans and contribute to their review.
* Promote self-care and independence, encouraging patients to engage in their own health management.
* Maintain accurate and timely documentation in line with GDPR and clinical governance standards.
* Participate in multidisciplinary team meetings and daily handovers.
* Escalate safeguarding concerns or deterioration in patient condition appropriately.
* Travel to patient homes across the PCN footprint (mileage allowance provided).
Person Specification
Qualifications
Essential
* Previous healthcare assistant experience within general practice.
* Competency in Phlebotomy.
* Full UK driving licence and access to a vehicle.
* NVQ Level 2 in Health & Social Care or equivalent experience.
* Excellent communication, empathy, ability to work independently
Desirable
* Use of clinical systems e.g. EMIS Web.
* IT Skills
* Knowledge of local health and social care pathways
