Reablement Support Officer, Part time (Reablement West Rehabilitation)
West Northamptonshire Council invites applications for the Reablement Support Officer role. The successful candidate will work with a range of community members each day, travelling to support them in regaining skills and confidence to enable greater independence. The role promotes choice and dignity, offering autonomy within a supportive team structure.
Responsibilities
• Travel to clients’ homes to deliver reablement support, including personal care, meal preparation, mobility and community access. • Undertake initial assessment and continually monitor progress, meeting clients after referral to assess needs and plan goals. • Case‑manage the reablement journey from assessment through to discharge. • Signpost clients to appropriate agencies such as Assistive Technology, Age UK and Northamptonshire Carers. • Work independently, handling unexpected situations, identifying problems and providing effective solutions. • Collaborate closely with the Reablement Team Manager and other health partners, such as the intermediate care team.
Qualifications
• Previous experience in a health or social care setting. • Current or willingness to work toward an NVQ Level 2 in Health & Social Care or equivalent. • Full driving licence and access to a private vehicle—travel countywide within Northamptonshire.
• Enhanced DBS certificate (cost covered by Council) and commitment to safeguarding children and vulnerable adults.
Shift Pattern
Cycle: 4 days on, followed by 4 days off
* Morning Shifts – 07:00 to 11:30 (2 per 4‑day block)
* Evening Shifts – 18:00 to 22:30 (2 per 4‑day block)
Total weekly hours: 16
Pay: £14.88 – £15.68 per hour, depending on experience. Mileage allowance up to 45p per mile.
