Your role will be as part of a team to support and care for patients to help them remain independent in their own homes. As part of our service, we provide an urgent crisis response supporting patients who are acutely unwell and preventing unnecessary hospital admissions. We also provide care and support for patients who are discharged from hospital to regain confidence, independence or who are awaiting a long-term package of care.
You will work closely with a team of healthcare professionals to support patients\' individual needs and promote patient health and wellbeing.
Main duties
 * To provide first visit assessments of patients in their own home, and ongoing care and support to the individual.
 * To provide subsequent visits and ongoing care and support as appropriate.
 * To assist and support patients and carers as appropriate, including helping with activities of daily living and administering medication as required, diet, maintaining personal hygiene, and assisting with toilet facilities as appropriate.
 * Support patients to meet their basic needs, including prompting and administering medication as required, supporting with dietary needs, maintaining personal hygiene, and using toilet facilities as appropriate.
 * To support patients with light household duties if the patient cannot do this due to being acutely unwell and does not have a support network to help with this.
 * Perform physical observations on patients using the necessary equipment (blood pressure monitors, blood sugar, and pulse oximeters) when appropriate or when advised to by a manager or member of the nursing team.
 * Ensure referrals to other agencies are made and effectively signpost patients to relevant agencies when appropriate.
 * Ensure relevant information, including all changes in a patient\'s condition, is passed to the registered nurse on duty. Ensure efficient hand over of visit allocations with Healthcare Support colleagues.
