Post: Advanced Practitioner Remote home triage England Hours: Part time hours or bank to be considered Pay: £27.49 - £30.63 depending on experience Reports to: Clinical Manager (and shift manager whilst on shift) Accountable to: Head of nursing - FCMS Base: Remote Working from home. Please note, there will be an expectation for the Induction to be onsite at head office (Newfield House, Vicarage Lane, Blackpool, FY4 4EW), and an expectation for a minimum of one day annually to attend head office. Hours of Duty: Flexible to meet the needs of the service may be anytime in the 24hr period Mon- Sunday which may include evenings and weekends. (Minimum of 4hrs a week expected to pick up). General Requirement: Must be a registered nurse or paramedic / AHP Role summary We are currently looking for ACPs that would like to do remote triage consultations for patients that may need further assessment to decide the next course of treatment. You would be expected to speak to a range of health professionals, including other nurses and paramedics that may be in pts houses awaiting advice, and to patients and relatives that may need advice or medication prescribing. You will need to be able to prioritise patients that are in the queue. We have services in various areas in the Northwest of England, and South Yorkshire. They include UTCs, 111, out of hours, CAS and APAS calls, meds query calls amongst others. Most calls are categorised according to need and you will be expected to clinically oversee them and select the most urgent from the queue and deal with it in completion, including generating prescriptions and sending by EPS as needed and closing with further advice, admitting pts or referring them to other services. Role The post holder will act as an autonomous practitioner working independently within their professional boundaries to assess, diagnose and treat the conditions of patients You will provide expert advice to patients, carers and colleagues and ensure the maintenance of clinical excellence over the phone. You will also be expected to undertake video calls as needed. The post holder will be responsible for promoting and facilitating a healthy lifestyle and provide preventative health care and health education advice. The role will primarily consist of telephone triage- providing advice and clinical assessment. You will also be required to request tests and follow up care with pts own GP as needed via message on completion of the call. You are expected to safeguard pts and report any concerns on shift to the shift manager and refer into safeguarding as required. You will pick up shifts via RotaMaster as required and may be requested to provide last minute short notice cover as your availability allows. The post holder will also contribute to administration and housekeeping duties within the locations. Duties and Responsibilities Recognise and work within own competence and professional code of conduct as regulated by the appropriate professional body Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures Prioritise, organise and manage own workload in a manner that maintains and promotes quality Deliver care according to local and national guidelines Be familiar with the use of different IT systems and have basic IT skills and EPS skills. Obtain and document consent for any treatment undertaken and keep the patient at the centre of his/her own care, ensuring full participation in decision making Remotely Assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing the services Interpret a range of diagnostic tests from the GP notes to hand and interpret clinical results to aid the consultation e.g. cytology, venepuncture, blood pressure monitoring etc. Work within all relevant policies and procedural guidelines e.g. governance and risk management Safeguarding responsibilities for adults and children and vulnerable groups, reporting concerns appropriately at the correct time on shift, and engaging with services as needed.
