Hampshire CC are seeking case workers in their CART (Community/Care and Adults Resource Team) Location: Fareham Rate: £23 (umbrella) Contract Length (End of March - possible extension) The CART Team is the contact centre for all Adult, Health and Care enquiries, dealing with nearly 100,000 enquiries a year by telephone, online forms and web chat. We operate 5 days a week. Our aim is to resolve a minimum of 85% of enquiries that come into CART. As a Case Worker you will be part of a supportive and friendly team working with other social/health care professionals managing incoming enquiries by telephone, online forms and webchat. You will assess resident's needs and provide them with support and guidance to enable them to maximise their wellbeing. Through your person-centred approach, you'll focus on the elements of care, support and treatment that matter most to the person, their families and carers. Using strengths-based practice, you'll focus on what a person can do, whilst finding innovative, alternative community resources to meet their needs.
