Overview
To assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
To manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action.
To use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
To actively take a personalised care approach and population centered care approach to enable shared decision making with the presenting person.
To complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.
To provide specialised clinical knowledge to assess patients, order and interpret diagnostics and initiate treatment.
To provide skilled team leadership to clinicians within the wider Multidisciplinary team.
To provide a high level of clinical expertise.
The post holder is an autonomous practitioner who will manage a caseload of patients, using evidence-based practice/patient focused principles to assess, plan, implement and evaluate interventions. So as a primary care network, with its member practices working ever closer together, it is important to establish shared core values which not only enables us to adapt, but to do so in the way that we all feel is right for our patients and staff. The outstanding contributions from Ryan, Roslea and Riverside surgeries in developing Bridgedales SHINE values is a key milestone and prepares the ground for the future.
Advanced Practice and Clinical Governance
As an advanced practitioner this post holder will work collaboratively across the PCN to play a crucial role in clinical governance, building capacity and capability and providing resolution and critical thinking to some challenging patient diagnosis or issues.
Whilst operating at an advanced practitioner level you will drive forward service improvements and clinical governance alongside seeing patients face to face and doing care homes ward rounds and home visits. The PCN will benefit from your level of knowledge, skills, experience, and leadership to build capacity and capability across the network.
Organisation Context
GP practices have been working together in our area for years and formed BridgedalePrimary Care Network (PCN). Bridegale Primary Care Network is made up of GP registered patient lists for Roslea Surgery, Riverside Medical Centre and the Ryan Medical Centre. We serve a community of around 30,000 people. We are small enough to provide the personal care valued by our people and large enough to have impact and economies of scale through better collaboration.
