As a Young Person's Health and Wellbeing Practitioner, you will be part of a bold new pilot supporting people aged 16 to 24 who are navigating substance use and the complex transition into adulthood. This is not just about treatment. It is about standing with young people as they move between child and adult services, and making sure no one slips through the cracks.

You will deliver person centred, trauma informed support through one to one and group sessions, in person and digitally. You might be based in A and E, visiting a care leaver service, or working alongside mental health professionals. Wherever you are, you will be focused on advocacy, continuity and harm reduction, helping each young person feel supported, not judged.

This is a one - year pilot role with the chance to shape how services respond to this often-overlooked age group. You will work creatively with partners, develop strong local relationships, and use your insight to help young people feel seen and supported as they find their feet.

About You

You bring empathy, energy and the kind of calm confidence that helps young people feel safe, respected and understood, especially when life feels uncertain or overwhelming.

You will bring:


* Experience supporting young people in community, education, health or social care settings
* A recognised Level 3 or above qualification in youth work, mental health, social care, counselling or a related field
* Confidence delivering one to one and group work that is person centred, strength based and trauma informed
* A strong understanding of safeguarding and how to create safe and supportive environments
* The ability to build positive relationships across services and work as part of a wider team
* Excellent communication and organisation skills, and the ability to manage your own caseload with professionalism and care

You will also reflect our values, acting with integrity, treating every young person as an individual, staying solution focused even when things are tough, and showing a consistent and reliable approach that young people can count on.
