We are delighted to be able to offer four Smoking Cessation Advisor posts across theCounty Durham area. Two successful candidates will be employed by South DurhamHealth CIC and a further two successful candidates will be employed by CentralDurham GP Providers (please specify your preferred location for work). (Termsand conditions may vary between both employers).


Main duties of the job

The role of the Smoking Cessation Advisor will be to betrained (if not already completed) as an NCSCT certified stop smokingpractitioner to support the delivery of evidenced based, equitable stop smokingservices to help to reduce health inequalities throughout general practices in CountyDurham. This will include the delivery of services within areas of mostdeprivation. It is essential that the postholder can work flexibly,independently and as part of a team.

This role will have a particular focus on supporting patientsfrom the County Durham area and the postholder will be required to work closelywith relevant partner organisations to best service the needs of patients in theCounty Durham area. While the core function of the role will be to deliverstop-smoking interventions, there will be an element of engagement and pathwaydevelopment to support the growth and sustainability of this workstream.


About us

South Durham Health CIC and Central Durham GP Providers are GP Federations and managing agents for eight Primary Care Networks in and around Easington, Sedgefield and Durham.

We also hold a number of Local Authority and NHS contracts, delivering additional services to our patient populations


Job responsibilities

This post will work under the line management of theFederation's Management team. The role will be varied, challenging, andrewarding as the services develop as part of the national 10 year plan.

You will be a motivated, passionate, organised, andproactive individual.

Key Responsibilities

Motivate and empower patients from County DurhamPractices to make changes to address their addiction

Work with key partners to establish effectivereferral pathways and raise awareness of stop smoking support within generalpractice setting.

Become embedded within general practice settings,building rapport with both prospective patients and staff within theorganisations.

Become a conduit between Smokefree County Durhamand organisations, to support the establishment of effective communication andfeedback processes.

Communicate regularly with partner staff toidentify training needs and deliver training around IBA and Level 2 deliverywhere appropriate.

Support the promotion of the Smokefree CountyDurham service within the South Durham practices.

Report against key KPIs in line with theSmokefree Generation Grant funding.

Provide regular, high-quality case studies from patients.

Explain to patients the principles of the StopSmoking Service to enable them to make informed choices

Deliver evidence-based stop smokinginterventions, these include one to one, drop in, group sessions, telephone, oron-line support, these must be in accordance with the NCSCT Standard TreatmentProgramme.

Work with other professionals to embed smokingcessation is part of routine patient care

Maximise the impact of smoking cessation onhealth gains and reduce health inequalities

To understand the analysis of health andequalities data including service data identifying local need to inform localservice need

Provide a high quality, efficient andcost-effective services throughout a diverse range of community and healthsettings

Lead peer support group sessions for successfulquitters following intervention to prevent relapse

Provide evidence-based education in all aspectsof smoking cessation and behavioural change as well on- going motivationalsupport

Work with individuals to set realisticsustainable goals

Ensure all patient and service records are up todate, accurate and satisfy the County Durham Health documentation policies

To engage and recruit potential clients atpromotional and community events

Provide administrative assistance to the servicein terms of patient contacts, appointment bookings and confirmations

Identify opportunities to develop and improveservices for patients

Maintain own knowledge of evidence-based methodsand integrate new research into practices

Assist in the organisation of the workloadensuring effective and efficient communication to other staff members

To conduct stakeholder engagement, identify andstrengthen referral pathways and identify potential co-working opportunities

Attend stakeholder meetings as required

Attend regular Stop Smoking Service Meetings

Attend training as required and keep up to datewith the wider aspects of Tobacco Control

Attend all training, internal and external tosupport the role

Experience

* Understanding of addiction
* Skills/knowledge to utilise computer software
* Ability to communicate effectively both verbally and in writing at all levels with individuals, groups and the public

* Ability to influence using skills and knowledge
* Ability to prioritise work tasks

* Understanding of service key performance indicators

Qualifications

* Training/Qualification or experience in a Health or Social Care setting
* Experience helping people with behavioural change and delivering health promotion
* Experience of managing own workload without day to-day supervision
* Experience of working proactively and unsupervised on a day-to-day basis
* Experience of supporting patients to improve health outcomes.
* Relevant professional qualification in health/ social care/public health/
* Experience of working within the community/Health & Social Care setting
