Job Summary
A Band 2 Support Worker will join the One Coventry Integrated Team to provide reablement support to patients experiencing an acute deterioration in health or functional ability. The service aims to prevent and reduce avoidable hospital admissions and facilitate early discharges by supporting patients through a reablement process. The role also involves identifying when reablement is not achievable and signposting to appropriate long‑term support.
Full valid UK driving licence and access to a vehicle for business use is required. Sponsorship cannot be supported.
Successful applicants must provide an immunisation and vaccination report from an occupational health provider or GP.
Main Duties
As a valued team member you will support patients with reablement while working in a multi‑professional base alongside Advanced Clinical Practitioners, Clinical Practitioners, Nurses, Care Coordinators, Physiotherapists, Occupational Therapists, Social Workers and other health and social care professionals. You will receive robust induction, appraisal, one‑to‑one meetings, regular training and clinical and managerial supervision.
Working Pattern
* Part‑time: up to 30 hours per week (5 days out of 7)
* Shifts: Early 08:00‑14:00 / Late 16:00‑22:00

Job Responsibilities
The role is part of a trial of a new Integrated Neighbourhood Team (INT) model. Responsibilities may evolve as the model develops but currently include:
* Providing reablement support to patients on a day‑to‑day basis.
* Collaborating with allied health professionals to maximise patient independence.
* Monitoring progress and escalating when reablement is not achievable.
