Ombersley Medical Centre is seeking an Advanced Clinical Practitioner (ACP) to join our innovative team in rural Worcestershire. Our goal is to deliver high-quality, personalized, patient-centered care in a safe environment, involving patients throughout their journey and improving their health outcomes.
The ACP role is new to our practice, working alongside the Duty Doctor to care for patients with acute illnesses and some chronic disease management through face-to-face and telephone consultations. Our team is encouraged to develop professionally in a supportive, organized, and innovative environment. Regular team meetings, a wide multidisciplinary approach, and community-enhanced services are integral to our practice.

Main duties of the job
The ACP will have the autonomy to assess, diagnose, and treat patients independently, providing timely care for acute and some chronic conditions, including history taking, assessment, diagnosis, prescribing, and care evaluation.
Responsibilities include making autonomous decisions based on assessments and test results, initiating and modifying interventions such as prescribing medicines, therapies, and lifestyle advice.

* Engage in peer review, seek feedback, and pursue ongoing professional development across clinical practice, leadership, education, and research.
* Provide clinics, maintain accurate records, and support team responses to emergencies and safeguarding.
* Understand policies related to substance abuse and health promotion.
Additional duties may include supporting infection prevention, participating in service initiatives, developing specialist interests, reviewing practices, and liaising with external agencies.
