Overview

Morley and District Primary Care Network (PCN) have an exciting opportunity for an experienced care co-ordinator to join their team. This role will help shape the local healthcare offer in Morley, Leeds. The suitable candidate should be passionate about improving primary care and comfortable working as part of a multi-disciplinary team across services.

Care co-ordinators proactively identify and support people, including the frail/elderly and those with long-term conditions, coordinating and navigating care across health and care services. You will work closely with GPs and practice teams to manage a caseload, acting as a central point of contact to ensure appropriate support is available and that changing needs are addressed. This includes creating personalised care and support plans based on what matters to the person.

Care co-ordinators may provide time, capacity and expertise to support people in preparing for or following clinical conversations, enabling them to be more involved in managing their care and making informed choices.


Key Responsibilities

* Work with people, families and carers to improve understanding of their condition.
* Support the development and review of personalised care and support plans to manage needs and achieve better healthcare outcomes.
* Encourage effective help-seeking behaviours and liaise with GP practices and other agencies.
* Provide a contact point for queries, appointments, and provision of clear information to enable informed choices about care.
* Coordinate and navigate care across primary, secondary, community, local authority, statutory, and voluntary organisations.
* Support transitions between secondary and community care, including follow-up appointments and broader system navigation.
* Refer to social prescribing link workers and health and wellbeing coaches as needed and involve clinical colleagues where there is unaddressed clinical need.
* Maintain regular liaison with a multidisciplinary team and participate in PCN meetings; keep records up to date in line with governance and data protection legislation.
* Record interventions and monitor the impact of care coordination on health and wellbeing; contribute to evaluation and quality improvement activities.


Person Specification

Experience
* Essential: Experience of working directly in a care co-ordinator role, adult health and social care, learning support, or public health/health improvement.
* Essential: Experience of data collection and using tools to measure service impact; understanding of how to use System 1; experience of working in primary care.
* Desirable: Experience with elderly or vulnerable people, multi-professional teamwork, and personalised care planning.
Other
* Essential: Access to own transport; basic knowledge of long-term conditions and their complexities (medical, physical, emotional, social).
Qualifications
* Essential: GCSEs A–C in English and Maths; DBS check requirements as part of recruitment.
* Desirable: Administrative qualifications.

Job Description

Responsibilities focus on enabling access to personalised care and support, taking referrals or identifying people who could benefit from care co-ordination, engaging in empathetic conversations with patients and families, and enhancing understanding of health management. You will develop knowledge of local health and care infrastructure and tailor support accordingly. You will update and communicate personalised care plans to relevant professionals and upload information to online care records with SNOMED coding where appropriate.
