Job Description:
An exciting job role has recently become available at the Wakefield Council. They are looking for a new member for their Adult Case Field Reviews Team.

Responsibilities:
* To undertake comprehensive needs assessments.
* To plan, implement and review care plans within the framework of the Care Programme Approach.
* To provide social work interventions to individual service users and carers as identified within the care plan.
* To contribute to the effective assessment and management of risk and protection of others. To liaise with the Probation Service and make referrals to the relevant MAPPA when appropriate.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
