Advanced Pharmacist Primary Care (Walk-In Centre)

We are seeking an enthusiastic and experienced Advanced Clinical Pharmacist to join our new GP Walk-In Centre team. This is a patient-facing role with a strong emphasis on same-day care, clinical decision-making, and independent prescribing.

You will work as part of a multidisciplinary primary care team, providing high-quality, accessible care for patients presenting with minor illnesses, long-term condition medication needs, and acute medication-related issues. This role is ideal for a pharmacist who thrives in a face-to-face clinical environment and is passionate about improving patient access and experience.

Key Responsibilities

· Conduct face-to-face consultations with patients attending the walk-in centre

· Independently assess, diagnose, and manage minor ailments and acute presentations within scope of practice

· Undertake structured medication reviews, including polypharmacy and medicines optimisation

· Prescribe safely and effectively as an Independent Prescriber

· Manage acute medication queries and medication-related problems

· Provide clinical advice and support to the wider GP and primary care team

· Ensure high standards of clinical documentation and governance

· Contribute to service development and quality improvement initiatives

Essential Criteria

· Registered Pharmacist with the General Pharmaceutical Council (GPhC)

As from 1st April 2026, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.

Due to legislative changes from 1 April 2025, this post may require a different level of criminal records check done than is currently the case. If the post is assessed as a 'regulated role', your appointment will be subject to joining the Protecting Vulnerable Groups (PVG) Scheme.
