Advanced Critical Care Practitioner

An exciting career opportunity has arisen for a dynamic forward thinking Advanced Critical Care Practitioner (ACCP) to join our highly motivated team working within the Cardiorespiratory and General Adult Intensive Care Unit (AICU) at Glenfield Hospital.

As an ACCP you will work alongside a supportive multidisciplinary team who have a real commitment to continuous improvement, shared learning, and best clinical practice, while being passionate about providing high quality care to our patients.

The ACCP will work within agreed protocols and guidelines exercising independent judgment in history taking, examination, assessment, diagnosis, Invasive procedures, clinical investigations and interpretation to manage and treat a diverse group of critically ill patients who may have highly complex presentations.

The successful applicant must have completed an MSc in Advance Clinical practice, be working in an Advance Clinical Practitioner role with a demonstratable portfolio of evidence and Intensive Care experience.

Main duties of the job

To be professionally accountable and legally responsible forall aspects of own work, including the management ofpatients in your care.

To have freedom to act within own scope of practice.

Participate in the provision of specialist advice on patientcare to the healthcare team and act as a specialist resourcewithin the service.

To maintain comprehensive and accurate assessment andtreatment records (written and electronic) in line with legal,professional and Trust requirements.

Understand professional accountability and legal frameworks for advanced practice

Job responsibilities

To accept clinical responsibility for undertaking the comprehensive assessment of acaseload of patients, including those with a complex presentation, in line with evidencebased practice, legislation, policies and procedures.

Using investigative and analytical skills; review and interpret all information available, utilising a systematic process of reasoning tomake a differential diagnosis.

Provide treatment plans for patients that are consistent with the outcome of assessment andthe most probable diagnosis using current clinical pathways where required in order tofacilitate timely, quality patient care and / or appropriate onward referral.Where appropriate, initiate diagnostic tests, interpret and act upon results and provideadvice to members of the multidisciplinary team on patient treatment. Evaluates the effectiveness of interventions/treatments and makes any necessary modification.

Practice autonomously; in planning and implementing treatment and care, undertakingadvanced clinical practice that has been agreed with the senior nursing, midwifery, therapyand medical teams within the specialty.

To use an extended scope of practice beyond own profession recognising any limitations inknowledge, referring to senior medical staff or Senior Advanced Practitioners when scope ofpractice is exceeded.

Maintain a safe environment for patients, relatives and staff, and adhering to infection prevention and control policies.

If registered as a non-medical prescriber, independently prescribe medication and fluidswithin current legislation and Trust policies.To accept referrals from and make referrals to consultants, GPs, and other healthcareprofessionals.

Review the patient and act accordingly, thereby reducing length of timepatients wait for intervention.

Improve the quality of the patient experience by identifying and meeting the individual clinicalneeds of patients. Act at all times in the patients best interests and as a patient advocate.

Provide specialist advice on patient care to the healthcare team and act as a specialist


Person Specification


Equality and Diversity

* Able to demonstrate a commitment to and understanding of the importance of treating all individuals with dignity and respect appropriate to their individual needs.


Education & Qualifications

* Registration on relevant part of the register with the NMC or HCPC with no restrictions on practice
* Master's degree level education meeting the requirements of the national framework for Advanced Level Practice
* Study/knowledge relevant to speciality.
* Have completed a Practice Assessor preparation day and have recent experience in supporting learners in the workplace and supervising their progress and assessing their practice.
* Post basic qualification in speciality
* Research module as part of Master's degree
* Independent non-medical prescribing (V300) if permitted within professional group
* Post graduate management / leadership qualification or willingness to undertake


Advanced clinical skills

* Evidence of developing advanced skills in history taking and physical examination skills within speciality area
* Highly developed communication skills
* Ability to assess, diagnose and action under pressurised environments
* Demonstrates expertise in complex decision making
* Autonomous management of own patient load (ward based or clinic environment)
* Autonomously assess, diagnose and develop management plans for urgently referred patients
* ALS/CALS
* FUSIC
* Evidence of ECMO experience


Experience

* Minimum of 2 years adult intensive care experience
* Experience of prescribing practice in the last 12 months
* Highly developed specialist experience across a wide range of intensive care units
* Experience managing own caseload of patients and autonomous practice
* Experience of teaching, assessing and mentoring of learners
* Experience in audit, research and development
* Proven line manager experience able to manage a team of healthcare professionals
* Evidence of autonomous practice
* Experience of service development
* Experience within a cardiorespiratory adult intensive care

* Self motivated and ability to motivate others
* Able to facilitate change
* Able to lead and work within a team

* Evidence of working under supervision and developing ability to autonomously assess, diagnose and develop management plans for urgently referred patients

* Ability to work on own initiative
* Vision to develop the service and the role of the ACCP
* Awareness of the national Framework
* Able to articulate advanced practice and professional accountability
