We’re Hiring: Case Assessor – Adult Social Care (Blackpool Area) Are you passionate about helping adults live independently and safely in their communities? We’re looking for a dedicated Case Assessor to join our Adult Social Care team in the Blackpool area. What you’ll be doing: Conducting assessments and reviews to support adults in maintaining independence. Developing, monitoring, and reviewing personalised care packages. Working closely with colleagues, providers, carers, and external agencies to deliver seamless care. Ensuring services comply with the Care Act (2014) and Mental Capacity Act (2005). Supporting and empowering individuals to make informed choices about their care. Advocating for vulnerable adults and ensuring safeguarding concerns are raised appropriately. What we’re looking for: Experience working in social care and inter-agency settings. Knowledge of adult social care, the Care Act (2014), and relevant policies. Excellent communication, negotiation, and IT skills. Ability to build strong relationships with service users and colleagues. Strong record-keeping, problem-solving, and decision-making skills.
