Thepurpose of the role is to lead improvements to maximise safe, cost-effectivebest practice in prescribing to improve the quality of patient care.

The post holderwill help patients to get the best from their medicines by switchingmedications to agreed and approved protocols, improving repeat prescribingprocesses in general practices, including promotion of repeat dispensing andonline ordering, minimising clinical risk, and aiming to reduce wastedmedicines.

The Pharmacy Technician will work under the supervision of the Primary Care Network (PCN) Pharmacist. We also offer dedicated support sessions from a GP within the practice.

You will be based in Bentley Medical Practice, Redcar, however there may be times when you are required to work across the local member practices of Redcar Coastal Primary Care Network.

We can be flexible with the hours for the role and are happy to consider any hours up to 37.5 per week with mutually convenient working days.


Main duties of the job

Support therole of the Clinical Pharmacist at GP practice level on the delivery of theClinical Pharmacy in General Practice model as detailed in the job description.


About us

Bentley Medical Practice is a GP Practice with a patient list size of 10,110 patients. We are situated in a modern purpose built building with free on-site parking for staff and visitors. We are a member practice of Redcar Coastal Primary Care Network.

Our friendly practice team consists of five GP's, two Advanced Nurse Practitioners, Practice Nurses, Healthcare Assistants and a full administration and reception team.


Job responsibilities

Support therole of the Clinical Pharmacist at GP practice level on the delivery of theClinical Pharmacy in General Practice model.

Support theClinical Pharmacist in Structured Medication Reviews (SMR) i.e., organisenecessary monitoring tests prior to SMR

Undertakepatient facing and patient supporting roles to ensure effective medicines use,through shared decision-making conversations with patients.

Assist in thedelivery of medicines optimisation and management incentive schemes and patientsafety audits.

Support theimplementation of national prescribing policies and guidance within the GPpractice and care homes. This will be achieved through undertaking clinicalaudits (e.g., use of antibiotics), utilising GP practice clinical systems toconduct such audits e.g., SystmOne. Supporting quality improvement measures andcontributing to all appropriate elements of the delivery of the DirectedEnhanced Service.

Carry out medicine'soptimisation tasks including effective medicine administration (e.g., checkinginhaler technique), supporting medication reviews and medicines reconciliation.Where required, utilise consultation skills to work in partnership withpatients to ensure they use their medicines effectively.

Carry out housekeepingwhilst in patient records e.g., dose optimisation and medicines synchronisation.

Promotion ofelectronic repeat dispensing and online ordering.

Report on keyperformance indicators for CPGP to demonstrate added value for patients andgeneral practice in accordance with NICE guidance.

Use andmaintain information systems and databases relevant to the position includingincident reporting, medicines optimisation, prescribing data, cost savings andpatient outcomes.

Work closelywith the PCN Clinical Pharmacy in General Practice lead.

Participatein practice meetings, patient participation groups, and other meetings toimprove engagement of the role of pharmacy technician within the CPGP model andto promote issues relevant to prescribing and medicines optimisation.

Liaise withother pharmacy stakeholders e.g., ICB Medicines Optimisation team, communitypharmacy, and hospital pharmacy teams.

Support theMedicines Optimisation team in the delivery of the Medicines Optimisation strategy.

Handlefunction specific information, which may be sensitive, complex, or confidentialand appropriately recording, transferring and/or coordinating such informationin accordance with the Data Protection Act; Caldicott Guidelines and theConfidentiality Code of Conduct.

To beresponsible for the organisation, planning and of own workload to meet set deadlines.

To undertake,participate and share the outcomes of clinical audits to facilitate improvementand changes in practice.

As instructedby the ICB Medicines Safety Officer, ensure that all major MHRA drug alerts,where appropriate and within level of competence, are acted upon withinpractices over an appropriate time frame and to advise on any medicationchanges that occur as a result.

To assist inthe monitoring of prescribing expenditure at individual practice level andpropose changes to ensure containment within identified resource.

To undertakelearning and development to ensure the required knowledge and skills for practice-basedwork.

To carry outother duties which are appropriate to the skills and competencies of the post holderand grade of the post as the priorities of the service change.


Person Specification


Qualifications

* Registered with the GPhC as a Pharmacy Technician
* Medicines reconciliation certificate or equivalent


Experience

* Experience of prescribing / clinical systems such as SystmOne and EMIS Web
* Recent previous experience within comparable role in General Practice
