We are looking for a motivated individual with excellent communication, interpersonal and organisational skills to join our team, to deliver care within an integrated way across the community.

Your focus will be on delivering day to day evidence-based nursing care and case coordination for children and young people aged 0-18 with acute, short- and long-term nursing needs. You will adopt a person-centred care approach, focusing on the individual needs of the child and their families, encouraging active engagement to empower and create independence whenever possible.

Main duties of the job

To achieve the best health and well-being outcomes for every child referred to the Community Children’s Nursing Team (CCNT).

To facilitate early discharge of children from hospital and reduce or prevent hospital re-admission.

To work together with parents to produce a joint plan of care to meet the needs of their child.

To enable and empower parents to look after their child with our support.

Person specification

Qualifications

Essential criteria

1. RSCN / RN Child with current registration
2. Preparation for mentorship course or equivalent
3. Evidence of continued professional development
4. Sound knowledge and demonstration of the use of evidenced based care.

Desirable criteria

5. Educated to degree level or working towards it
6. Childrens Community Nursing qualification
7. Non-medical prescribing qualification

Experience

Essential criteria

8. Post registration children’s nursing experience at Band 5
9. Previous experience of working with children with complex needs.
10. Experience of successful working across organisational boundaries
11. Experience of assessing and supervising nursing students

Desirable criteria

12. Leadership of an area
13. Awareness of budget and stock control
14. Health promotion experience
15. Previous experience of line managing junior members of staff

Skills

Essential criteria

16. High standard of clinical expertise
17. Ability to work independently and within a team
18. Excellent organisational skills
19. Adaptable and able to deal with changing workloads
20. Decision making skills
21. IT Literate
22. Ability to plan and deliver training sessions for parents/MDT with a range of different abilities and to assess and evaluate their learning through the use of competencies on a regular basis.
