We are delighted to offer an exciting secondment/fixed term contract opportunity for a passionate and experienced clinician to join our Community Mental Health Rehabilitation Service as a Clinical Lead.
Based at Engineers House on the St Austell Hospital site, our team provides county-wide support to individuals recovering from complex and enduring mental health conditions. Working in close partnership with Fettle House, our inpatient rehabilitation facility, we offer a seamless and person-centred pathway that supports patients in transitioning to independent living within the community.
As part of Cornwall Partnership NHS Foundation Trust, our Community Rehabilitation Team collaborates closely with carers, families, and a range of specialist services including the Early Intervention Team. We also play a key role in facilitating discharges from acute wards and work alongside housing providers, education services, and pharmacy teams to ensure holistic and sustainable recovery.

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
Main duties of the job
* Provide specialist skills, knowledge, and expertise in the assessment, care, and management of all patients referred to the Community Mental Health Rehabilitation Team (CMHRT).
* Conduct comprehensive assessments and collaboratively develop, implement, and evaluate individualised care plans with service users, carers, and professionals across health and social care.
* Undertake thorough risk assessments and apply appropriate risk management strategies to ensure safety and wellbeing.
* Demonstrate advanced and specialised knowledge in mental health care, contributing to high standards of clinical practice.
* Empower service users by enhancing their understanding of their mental health conditions and providing accessible information and health promotion resources.
* Work in partnership with families and carers, offering support to help reduce psychological distress associated with the caring role.
* Ensure robust implementation of risk management procedures, including safeguarding policies for both adults and children.
* Respond to complaints, concerns, or queries from service users and others, initiating local resolution in line with Trust policies and procedures.

Person Specification
Education / Qualifications and Relevant Experience (Essential)
* Current professional registration - NMC/AHP
* Professional qualification - RMN/AHP
* Management / Leadership qualifications or current study or equivalent experience
* Substantial experience of working in adult mental health settings
Education / Qualifications and Relevant Experience (Desirable)
* Solution Focussed training/ experience
* Additional training in mental health assessment
* Experience of working with substance misuse
