Senior OT / Pathway determiner - Community Therapy Services

Don't miss out on this fantastic opportunity to join our Rushcliffe Community Therapy Service as an Occupational Therapist working in the community. If you want to be part of a forward thinking and innovative service, then this is the opportunity you may have been looking for.

In your new role you will work closely with Physiotherapists, Assistant practitioners, support staff, district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes.

We are working towards a 7 day service - 8am to 8pm on rota basis. Self rostering is encouraged.


Main duties of the job

An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management, palliative care support and support timely discharges from our acute Trust partners.

You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care. You will be visiting patients within their own homes using a goal-orientated focus, to maximise patient\'s independence, safety and quality of life.

The post will involve cross site working and community visits.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Job responsibilities

You will manage your own caseload in the community. As part of the integrated care team you will work closely with the rest of the MDT to provide the best possible outcomes for our service users in Rushcliffe.

You will have a degree/diploma in Occupational Therapy & be HCPC registered. You should possess excellent interpersonal and time management skills, be a good communicator and able to prioritise and work well as a member of a team. An induction period and supernumerary support will be given. Ideally you will have experience of working in the NHS in the UK and being part of our healthcare services.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

Physical Requirements

* Able to carry out manual handling activities
* To contribute to policy and service development in order to redesign services in response to national and local agenda
* To continue to work in emotional stressful situations (e.g. Palliative Care) providing psychological support to patients, carers and colleagues


Qualifications

* Degree in Occupational Therapy
* Current Health and Care Professions Council (HCPC) Registration
* Post registration study
* Involvement in research/audit and training


Training


Experience

* Substantial post registration broad clinical experience as a band 5 (or equivalent) Occupational Therapist or Previous community experience
* Experience of working with older adults.
* Previous community experience
* Experience of working and supervising Assistants


Knowledge

* Understanding of relevant national and local policy and integrated working.
* Understanding of long-term conditions agenda.
* Member of Special Interest group
* Able to educate and manage assistants


Skills

* Basic IT Skills
* Good prioritisation and organisation.
* Effective team player
* Flexible approach to meet the needs of the service/locality, potentially working from different bases according to the needs of the service.
* Proven Leadership skills


Contractual Requirements

* Able to work flexibly over seven days


Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
