Main duties of the job Are you passionate about making a meaningful difference in the lives of children and young people and their families living with diabetes? We are seeking a compassionate and motivated Paediatric Diabetes Family Support Worker to join our Paediatric Diabetes Multidisciplinary Team which includes Consultants, Paediatric Diabetes Nurse Specialists, Dietitians, and Clinical Psychologists. You will provide personalised support to children and young people with diabetes, and their families, both in hospital and community settings. You will play a vital role in promoting well-being, independence, and positive lifestyle choices, while helping families navigate challenges related to health, education, housing, and social care. Reports to Lead Paediatric Diabetes Nurse Specialist and Lead Clinical Psychologist. Essential Detailed job description and main responsibilities Educated to GCSE (or equivalent) standard, with a pass in English and Maths NVQ Level 2 or 3 in Care or equivalent Evidence of recent learning or development in health You Will Require Excellent communication skills with the ability to interact appropriately with patients, relatives, team members and other staff groups. The ability to work flexibly across core hours (and on rare occasions, outside core hours) during the week to meet the needs of the service. To travel between hospital and community bases such as patients homes, community hubs and between hospital sites. Experience in working with children, young people, or families in a health, social care, or community setting, including the provision of holistic and tailored support.
