An opportunity has arisen for an enthusiastic Mental Health practitioner/CPN to work within the East and Mid Ross Older Adult Community Mental Health team in the Highlands which offer a wide range of outdoor activities for people and families. The successful candidate will work collaboratively with older adults and their carers experiencing both functional and organic mental health problems.
You will be required to carry out assessments, care planning/formulations and reviews and act as a care co-ordinator where appropriate for patients in the above area.
The post holder will be skilled and confident in the delivery of recovery-based person centred approaches, having the ability to autonomously manage their workload safely and effectively, demonstrating enhanced skills such as clinical risk assessment/management and positive risk taking. Evidence of delivery of or facilitation of training such as The Essentials in Psychological Care, CST would be a desirable recommendation for this post as this role will also provide input as Care Home Liaison to support the staff with patients who present with stressed and distressed behaviours as a result of their illness.
You will demonstrate a broad range of clinical therapeutic work as well as being able to work both independently and within the multi-disciplinary team setting. You should possess sound clinical knowledge and be able to demonstrate a wide range of clinical skills - including the ability to offer interventions and deliver safe, effective care to a diverse patient population.
The successful candidate will exhibit enthusiasm regarding multi-disciplinary working, and feel confident in working as an integral member of the team to ensure safe and effective clinical practice is delivered to enhance the patients and carers experience of care.
The opportunity for further training within the CMHT is available for example Non Medical Prescribing. The post holder will have regular clinical and managerial supervision from the Team Lead on all aspects of their role.
