Overview
An exciting and challenging opportunity has arisen for an enthusiastic, motivated and forward thinking advanced clinical pharmacist to join the Rheumatology team, made up of Consultants, CNS and an Advanced Pharmacist. The post is full time, but part time and job share applications will be considered. This post will require working weekends and working on call.
This role requires you to prescribe and manage homecare prescriptions for rheumatology patients receiving biologic treatment. You will be required to work closely with the homecare department to ensure that our rheumatology patients receive their biologic treatment in a timely manner with appropriate monitoring.
This will also require management of biologic drug shortages or biosimilar switches, to optimise cost effective prescribing within rheumatology. You will be required to answer homecare queries relating to the management of our rheumatology patients and support the multidisciplinary Rheumatology team with medicine queries and prescribing.
The role will require you to write and review rheumatology specific treatment guidelines to ensure the safe prescribing and administration of medicines within rheumatology and cost effective use of treatment.
The role may also incorporate the management of patients prescribed disease modifying anti-rheumatic drugs until stabilised and liaising with primary care providers to ensure the safe and effective prescribing of DMARDs in the community through shared care prescribing.
Responsibilities
* Prescribe and manage homecare prescriptions for rheumatology patients receiving biologic treatment, in collaboration with the homecare department.
* Monitor and ensure timely administration of biologic therapies and related monitoring.
* Manage biologic drug shortages or biosimilar switches to optimise cost-effective prescribing within rheumatology.
* Respond to homecare queries relating to the management of rheumatology patients and support the multidisciplinary team with medicine queries and prescribing.
* Create and review rheumatology-specific treatment guidelines for safe and cost-effective use of medicines.
* Manage patients prescribed DMARDs until stabilised and liaise with primary care to support safe prescribing in the community via shared care.
* Demonstrate excellent communication, interpersonal and time management skills; strong decision making and problem solving abilities.
Requirements / Qualifications
* Qualified pharmacist with advanced clinical experience in rheumatology, capable of prescribing and managing homecare prescriptions for patients receiving biologic treatment.
* Excellent communication, interpersonal and time management skills; strong decision making and problem solving abilities.
* Ability to work weekends and on call; flexible to part-time or job share arrangements.
* Desirable Welsh language ability; English and/or Welsh speakers are welcome to apply. The health board supports Welsh language learning options during employment.
