Key Duties and Responsibilities
- Support PCN Pharmacy Technicians
- Conduct clinical medication reviews
- Provide medication reviews for individuals in Care Homes
- Conduct patient-facing clinics for those with long-term conditions
- Receive referrals and provide advice to patients in need
- Manage common/minor/self-limiting ailments
- Provide medicine information to practice staff and patients
- Offer support, education, and training to others
- Support reduction in unplanned hospital admissions
- Manage medicines post-hospital discharge
- Support public health campaigns
- Safety netting in patient contact
- Risk stratification and patient cohort identification
- Information management and service development
- Medicines quality improvement and safety
- Undertake clinical audits of prescribing
- Implement changes in accordance with MHRA alerts and guidelines
- Ensure compliance with CQC standards
- Support Practices with CQC inspections
- Communicate key updates to Practice colleagues
- Support implementation of local and national guidelines
- Monitor Practice prescribing against local dashboards
- Collaborative working within the PCN Network
- Participate in multi-disciplinary team meetings
- Utilize sensitive communication styles
- Build positive working relationships
- Engage in professional development and mandatory training
- Maintain professional registration and indemnity arrangements
- Take responsibility for health and safety
