Patient Flow Coordinator – Oak Unit
We are looking for a Patient Flow Coordinator to support admissions and discharge planning on the Oak Unit (formerly Ward 2), a 34-bed frailty assessment unit receiving direct admissions from A&E and OAU at Wythenshawe. The role is integral to ensuring timely and safe discharge of frail adults within the Trafford and Manchester area.
Key Responsibilities
* Assess patients’ usual/pre-admission support in the community and plan discharge arrangements.
* Liaise with the multidisciplinary team, patients and families during admission to confirm the right support is requested and secured for discharge.
* Regularly communicate with multi‑agency services, including Social Services and the private sector, to ensure patient safety beyond discharge.
* Support the ward manager and multi‑disciplinary clinicians in managing patient flow and operational needs.
* Maintain accurate records of admission and discharge planning activities.
Qualifications & Traits
* Compassionate and patient‑focused approach to the care of frail adults.
* Excellent teamwork, communication and flexibility to meet the changing needs of the ward.
* Demonstrated ability to work in a dynamic environment and support a new role focused on patient flow.
* Experience or knowledge of discharge planning is desirable.
