Physiotherapist/Occupational Therapist Community Falls/FLS team

Anexciting opportunity has arisen for a specialist Physiotherapist/Occupationaltherapist to join the community falls and FLS team who now work collaborativelytogether. This role is predominately to work in the falls team and would workacross both services as required. The role involves assessing and treatingpatients who have had falls or risk of falling. You will be carrying out acomprehensive assessment looking falls prevention and rehabilitation.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities
Client Group: Adult rehabilitation to community basedpatient population over 5-7 day service as required
To be an autonomous practitioner with the freedom to act tobe personally, professionally and legally accountable for the services youprovide to patients in your care and for those Associate Practitioners, Careand Therapy Support workers and To perform advanced therapeutic assessment of patients withdiverse presentations and sometimes highly complex physical conditions; toprovide a diagnosis, develop, and deliver individualised treatment programme
To hold responsibility for own caseload and be responsiblefor a defined area of the service or a particular patient type, working withoutdirect supervision.
To be operationally managed on a day to day basis by yourdesignated clinical lead, and will receive formal supervision from a B7therapist either within group or individual sessions.
To be supported by the designated Clinical Lead to carry outyour duties via informal supervision and guidance on an as needs basis,dependent on your level of experience and complexity of patients under yourcare.
To work within established Service / Trust policies andprocedures and contribute / propose changes for the development of operationalprocedures / working practices relevant to the team and services it provides.
To be responsible for providing formal clinical supervisionand support to lesser experienced therapists, plus Assistant Practitioners andCare support workers. This includes personal and professional development,including appraisal as well as support for case management beyond their level ofknowledge and experience. Informal support will also be given on a day to daybasis to assist with caseload management.
To provide supervision, education and assessment of therapystudents; this would be to a graduate standard and involve working with universitiesto ensure the standard of practice and teaching meets the standard set by thedegree level qualification
To undertake evidence based audit to further own and teamsclinical practice within the specialism. To make recommendations to theClinical Lead of service that may lead to implementation of specific changes topractice or contribute to service protocols

Experience
* Comprehensive and relevant qualified experience in physical rehabilitation including recent general rehabilitation experience.
* Experience of collaborative working within multidisciplinary teams
* Evidence of communication skill within a wide variety of settings
* Ability to travel and carry small items of equipment on a daily basis to fulfil the requirements of the post
* Experienced in staff appraisal
* Experienced in working in a variety of settings
* Experience of crisis management

* In depth and up to date knowledge and skills in the assessment and treatment of a wide range of physical disabilities

Qualifications

* An understanding of County Durham and Darlington NHS Foundation Trust, its culture, strategic direction, achievements and NHS standing
* Involvement in specific interest groups
