The respiratory nursing team are looking for a motivated and enthusiastic Health Care Support Worker to join our friendly multi‑disciplinary team. Our current service model is expanding to include a respiratory hospital at home and specialist respiratory ward. Our aim is to provide excellent person centred holistic care to our service users and their families.
Respiratory Hospital at Home is designed to facilitate early supported discharge of patients from acute medical beds and prevent hospital admissions of patients with respiratory illnesses in the community, so creating a virtual ward.
The post holder will be responsible for the implementation of care and on‑going assessments of patients with chronic respiratory conditions, therefore the successful candidate should have a genuine interest in caring for patients with a respiratory condition and demonstrate a high level of independent working and good communication skills.
As the role includes home visits the ability and willingness to travel throughout the area of service delivery is essential for this post.
The successful candidate will be required to have an excellent telephone manner, knowledge of various IT systems and be ablet to confidently communicate with the wider multidisciplinary team. They will work closely with the Respiratory Nurse Specialists.
