Main duties of the job
The Practice Nurse is responsible for providing nursing services to the practice population, within the boundaries of the role. Successful Practice Nurse will support patients to be healthy, monitor patients with long-term conditions, and deliver health prevention, promotion and screening activities. This will include but is not limited to
* Management of chronic disease
* Cervical Cytology
* Childhood Immunisations
* Administer a range of adult vaccinations
* Wound care
* ECGs
* Actively participate in achieving QOF and Enhanced Services targets and recall demands
* Participate in practice audit as directed by the clinical lead/practice manager
* Support and participate in shared learning within the practice
* Play an active part of the clinical team at the practice attending regular nursing, education and other meetings

Job Responsibilities (this list is not exhaustive):
* Chronic disease management – special interest in Asthma / COPD / Diabetes or Coronary Heart Disease and Contraception
* Cytology
* Family planning
* Wound care
* Travel clinic and vaccinations
* ECGs
* Asthma/Peak flow readings/Spirometry
* Routine immunisations / Childhood immunisations
* Requesting pathology tests, e.g., urine culture, swabs
* Following agreed clinical protocols with referral to Lead Nurse or GPs as appropriate
* Deliver opportunistic health promotion where appropriate
* Participate in practice audit as directed by the clinical lead/practice manager
* Participate in local initiatives to enhance service delivery and patient care
* Support and participate in shared learning within the practice
Person Specification
Qualifications
* NMC Registered Adult Nurse with 3+ years experience
