Opportunities exist for you to work with rapid access from our referral centre, bed-based reablement, Home First' and domiciliary based intervention, so there's plenty of scope for your professional development.

As a B5 you will need:
* Relevant experience of working as part of an integrated multi-disciplinary team
* Excellent communication and IT skills.
* Experience of working in people's homes
* Be highly motivated to provide evidence based practice.
