Overview
We are looking to recruit a Support Worker to join our Community Offer Plus team to assist the 7-day week service, with day-to-day operations. The aim of the service is to provide a range of support for people to live independently upon hospital discharge, to support discharge back home including transport, and to provide up to 12 weeks of low-level support to Sandwell residents aged 18 plus including information, advice, guidance and signposting.

We work collaboratively with public, private and voluntary organisations, educational settings and other partners to design and deliver sustainable innovative solutions to improve the lives of children, families, young people and adults from early years to end of life care.
Responsibilities
Provide day-to-day support as part of the Community Offer Plus team to assist the 7-day week service. Support individuals to live independently after hospital discharge, including transport where needed, and deliver up to 12 weeks of low-level support with information, advice, guidance and signposting to Sandwell residents aged 18 plus.
