Job overview
The post holder will be an experienced general practice pharmacist with an independent prescriber qualification. They will be supported and work alongside a team of pharmacists and pharmacy technicians. In this role they will be supported by practice GPs and the PCN Clinical Directors for management and mentoring
The post holder will and provide clinical pharmacy across the PCN proactively and competently managing acute and long-term conditions, prescribing/referring where appropriate to do so.

Main duties of the job
The post holder will be holding a leadership role within Hounslow Health Primary Care Network (PCN), which comprises ten member practices and will work within their clinical competencies as part of a patient facing PCN multi-disciplinary team to:
1. Provide expertise in clinical medicines management
2. Provide face to face structured medication review
3. Manage long term conditions
4. Manage medicines on transfer of care and systems for safer prescribing
5. Manage repeat prescription authorisations and acute prescription requests
6. Assist with provision of Enhanced Services such as Diabetes and Care Homes
7. Assist with the provision of PCN Anticoagulation clinics
