Older People and Hospital Social Worker
Location: North Wales
Pay rate: £29 per hour
We currently have a short-term vacancy for a Social Worker to work in our Older People & Hospital Social Work Services. The suitable candidate will be expected to deliver a high-quality social work service to Older People who are deemed vulnerable or needing support. You will work with citizens, carers, and families to provide support, develop resilience, and promote independence, ensuring that the citizen, families, and carers are at the centre of decision-making about their lives and desired outcomes.
Working in accordance with social care legislation and local government requirements, and guided by the values and principles of social care, you will support the delivery of person-centred, safe, and effective social care services. Using your professional knowledge and experience, you will make assessments and decisions about the most appropriate interventions. Applying your significant social work experience and excellent communication skills, you’ll establish and maintain relationships with service users and other agencies to ensure services meet identified needs.
You’ll be committed to ensuring positive and sustainable results from your interventions. Using best practices, you’ll aim to provide the best outcomes for service users. Most importantly, you’ll have a passion for supporting children and their families to thrive and flourish. Our priority is to protect the most vulnerable and to be as efficient as possible.
Perfect candidate will be:
* Qualified Social Worker registered with Social Care Wales
* Experienced in working with children and families, including vulnerable groups
* Creative, flexible, and forward-thinking in approach
* Constantly striving for continuous improvement to achieve the best outcomes for children
* Able to use excellent mediation and negotiation skills on behalf of service users
* Highly skilled at building meaningful relationships with children and families to engage them in assessments and plans
* Committed to providing high-quality services
* Experienced in working within multi-disciplinary, interagency teams
* 4+ years qualified
In this role you will:
* Use your skills, knowledge, and experience to assess risks, protection needs, and social care requirements
* Manage an agreed caseload
* Utilize specialist communication skills to achieve optimal outcomes for service users
* Devise individual care plans, considering diversity, dignity, and empowerment
* Offer advice and support to unqualified staff and students for their development
