We are looking for a dynamic Nurse to join the Poole Intermediate Care Service – a multidisciplinary team that prevents hospital admissions and secures early hospital discharges.
Responsibilities
* Demonstrate leadership.
* Provide a high standard of holistic assessment and appropriate timely reviews.
* Plan specialist care and/or rehabilitation programme to promote and sustain independence and well‑being for the individual in their own home.
* Deliver nursing, rehabilitation and care programmes for adults, many of whom have complex clinical and social needs, with the aim of improving independence, function and quality of life.
Requirements
* Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service may present a valid DBS certificate instead of requiring a new check.
* This is a fixed‑term role (initially 5 months, likely to be extended) and, for those already in NHS service, a secondment may be offered.
