Overview
Compass House Medical Centres are seeking a dedicated Carer Support Worker to provide support, guidance, and assessments for unpaid carers in our community. This role focuses on improving carers health and wellbeing, ensuring they are aware of and can access appropriate services and resources. You will work closely with GP practice teams, social prescribers, and external agencies to identify carers needs and deliver person-centered support.
Key Responsibilities
* Undertake Health and Wellbeing Checks and assessments for carers, identifying needs, outcomes, and Care Act eligibility.
* Provide guidance and signposting to community resources and specialist services.
* Support carers contingency planning, coding, and the Carers Passport initiative.
* Liaise with statutory and voluntary agencies, advocating for carers when required.
* Contribute to the practices achievement of GP Carers Quality Markers and compliance with national and local initiatives.
* Deliver Carers Awareness Training to practice staff
* Maintain processes for reporting statistics while ensuring confidentiality and consent.
* Participate in local projects and initiatives that enhance carers support.
Person Specification / Requirements
Essential
* Experience working with carers, vulnerable adults, or in health/social care settings.
* Knowledge of Care Act, carer assessments, and local support services.
* Strong communication, interpersonal, and organisational skills.
* Ability to work independently and collaboratively.
* Empathy, patience, and commitment to supporting carers wellbeing.
* Experience with project work, data reporting, or community engagement is desirable.
Desirable
* Experience of working within Health or Social Care.
* Assessment skills
* Understanding of key issues affecting Carers.
* Experience of working with Carers or personal experience of a Caring role.
* Committed to putting the Carer at the heart of work and to using Carer experience to shape support
* Committed to whole family working
* Committed to inter-agency partnership working
* Committed to supervisory process / annual appraisal
* Proficient IT and recording skills (Word/ e-mail/ databases)
* Ability to work on own initiative and as part of a team
* Ability to prioritise and manage own workload
* Reliable and flexible with a creative approach to problem-solving
* Ability to collect and collate data as required.
