We are excited to offer a fantastic opportunity for an experienced Band 6 Community Nurse or a developmental role for an experienced Band 5 District Nurse or Community Nurse to join our team within Adult Community Services in Surrey.
As a Community Nurse, you'll play a pivotal role in the delivery of care within our Integrated Care Teams (ICTs) across Surrey Heath, working alongside multiple internal and external services.
Your expertise and compassion will make a significant difference in improving the health outcomes and quality of life for individuals under your care.
This role covers the Surrey Heath locality, and you will be expected to travel across the area as required; therefore, having a driving licence and access to a vehicle is essential.
The community nursing team works from 8am to 8pm, 365 days a year. Our shifts include a mixture of 8am to 4pm, 9am to 5pm, and 12pm to 8pm. You would only do one late shift a week and join the rota to work one weekend in four.
Main duties of the job
Within this role, you will:
* Assess, plan, implement, and evaluate programmes of care to meet the needs of clients and carers in primary care settings, including patients' homes, residential homes, and GP surgeries.
* Manage chronic disease for housebound patients, making autonomous clinical decisions and undertaking health promotion activities with patients, carers, and families.
* Manage admissions and discharges, assessing and advising patients, relatives, and GPs.
* Deliver skilled nursing care in accordance with national and local priorities and the NMC Code of Professional Practice.
* Monitor ongoing interventions and initiate changes as required.
* Oversee medication regimes, assess effects, and advise on safe storage and disposal.
* Ensure the correct use of aids and equipment supplied for patients and carers.
* Manage care for terminally ill patients at home, including complex pain and symptom management in liaison with the MDT.
* Supervise staff directly and participate in the orientation and induction of new staff.

Job responsibilities
You will be a registered Nurse with NMC/1st level registration, preferably with some community experience. You should be able to work independently and as part of a multidisciplinary team. Strong communication, organizational skills, and a flexible approach are essential to provide positive patient experiences throughout their healthcare journey. A full UK driving license and access to a car are required.
You should be confident using IT systems and have working knowledge of Microsoft Office.
