Overview
Our Front Door is a new, unique and exciting service development that is at the very beginning of a young person’s journey in seeking support.
We will undertake triage, deliver initial assessments to families and young people referred to our service, engage with them creatively (through consultations or brief interventions as appropriate) to plan the support they need, and liaise with the system around the family to ensure a holistic approach.
Responsibilities
* Complete first line triage to determine need including risk and urgency of care, then signpost to the most appropriate service.
* Assess the current mental health need of children and young people referred to CYPMH using appropriate means, including face-to-face in clinic or home visits.
* Identify areas of need for children and their families and assist them in accessing appropriate services.
* Develop and utilise specialist knowledge about working with children and their families.
* Work as part of a team to coordinate the delivery of care.
* Ensure that any plans and recommendations have taken into account the views of the child and carers.
* Undertake risk assessment and risk management for young children and their parents/carers.
