PCN Manager & Digital Transformation Lead

The PCN Manager will support the PCN to develop and implement service improvement and transformation work, aligned to local and national priorities. This includes building on current primary care provision and working proactively with the wider network to ensure we can offer our local population coordinated and more integrated healthcare. The role entails building effective working relationships, working closely with the Clinical Director, engaging stakeholders across different organisations and disciplines, coordinating work-streams, collating and monitoring risks and issues.
The PCN Manager will coordinate the successful delivery of all contractual requirements. You will work to develop the PCN strategic plan and provide project management support to deliver services in line with the PCN Network Contract DES and the PCNs priorities.
The PCN Manager will provide line management to all PCN employed staff to ensure they are supported and co-ordinated within the PCN. This duty will include line management to clinical and non-clinical staff, however clinical supervision will be provided by an appropriate registered healthcare professional.
Main duties of the job
* Ensure all relevant national strategies, policies, targets, and frameworks are fully implemented.
* Clarify objectives and priorities for network delivery in line with operational capacity, financial parameters and contract requirements.
* Provide oversight for the delivery of PCN contracts and specifications, including enhanced services and the Impact and Investment Fund through the monitoring of key performance indicators, data quality and compliance.
* Work with the PCN's Practice Managers on change and continuous improvement initiatives; leading and coordinating projects as required to ensure successful delivery.
* Maintain records of PCN projects including objectives, milestones and progress.
* Proactively explore potential new projects and project partners.
* Ensure projects meet the needs of the PCN population and link to relevant contracts (DES/LES) when required.
Person Specification
* Strong knowledge of General Practice regulation and information governance
