Job summary

The Acute Liaison Nurse will act as a clinical expert and professional resource to the multi-professional team, patients with a learning disability, their families and carers across the Trust.The post holder will ensure the delivery of high quality, clinically effective care, utilising research, and collaborating with colleagues in everyday practice and work in partnership with the MDT to enhance professional standards of care across the speciality.


Main duties of the job

* The post holder will plan and deliver care in accordance with the Trust's committment to deliver high quality care .
* The post holder will work in collaboration with mainstream hospital services to enable open and easy access to healthcare services for people with learning disabilities.
* The post holder will work alongside health professionals and senior hospital staff to achieve a positive experience and clinically appropriate outcomes, for patients with a learning disability.
* The post holder will initiate and support person centred approaches to the delivery of care within healthcare services.

* Registered Nurse Learning Disability Nurse - with current NMC Registration and no restrictions on practice
* Degree in Nursing, leadership or management or relevant subject or evidence of practical application of expertise at this level
* Evidence of post qualification study and updates.
* Evidence of continuing personal and professional development
* Knowledge of the mental capacity acts and DoLs compliance

* Master Degree

* Experience caring for learning disability patients
* Demonstrable experience of multiagency working
