Role Overview:

To have a responsibility for a caseload, including undertaking assessments, developing Care & Support Plans, and revising them as appropriate once they are implemented. To improve life opportunities for Service Users, Carers and Families using social work methods and models. To continue to develop professionally, and be accountable for the delivery of, and promote, social work good practice.

Key Responsibilities:

* Undertake holistic assessments of service users, carers, and familiesincluding risk assessmentsand develop, implement, and regularly review person-centred Care and Support Plans in line with statutory duties.
* Coordinate and manage transition planning for young people moving from Children’s to Adult Services, promoting independence, well-being, and informed decision-making through Self-Directed Support and Individual Budgets.
* Safeguard vulnerable individuals by proactively identifying and responding to safeguarding concerns, crisis situations, and legal interventions, ensuring compliance with relevant procedures and legislation.
* Maintain accurate, high-quality records on case management systems (e.g., MOSAIC), produce professional reports, and protect the confidentiality of personal information.
* Work collaboratively with multi-agency partners, advocate for service users, and contribute to service improvement through supervision, performance reflection, and ongoing professional development.

Key Requirements:

* Experience in conducting accurate and sensitive assessments of Service Users, Carers, and Families to develop tailored Care and Support Plans.
* Proven ability to implement, monitor, evaluate, and update Care & Support Plans using person-centred planning principles and relevant procedures.
* Skilled in prioritising workload, setting objectives, and managing case records, ensuring timely and accurate documentation.
* Experience working collaboratively within multi-disciplinary teams to deliver comprehensive social work services.
* Strong analytical and report-writing skills, with experience in collating information and producing reports suitable for formal settings.
