We are now looking for a dedicated Care Coordinator to join our team — someone who is organised, compassionate, and ready to play a key role in delivering outstanding care in the community.
The Role
As a Care Coordinator, you will be responsible for planning and coordinating care services, ensuring our clients receive safe, reliable, and high-quality support. This is a varied and hands‑on role, which includes participating in an on‑call rota and delivering care when required to support the service.
Key Responsibilities
* Coordinate and schedule care staff to meet client needs
* Ensure care visits are delivered efficiently and on time
* Maintain accurate records and care plans
* Communicate effectively with care staff, clients, and families
* Support with onboarding and ongoing support of care workers
* Participate in the on‑call rota to support out‑of‑hours service delivery
* Deliver care in the community when required to ensure continuity of service
* Ensure compliance with company policies and regulatory standards
What We're Looking For
* Previous experience in a care or care coordination role
* Strong organisational and problem‑solving skills
* Excellent communication and interpersonal skills
* A flexible and hands‑on approach
* Ability to work under pressure in a fast‑paced environment
* Compassionate and person‑centred mindset
* Full UK driving licence (essential)
* Willingness to participate in on‑call duties and deliver care when needed
