Lipid Management and Secondary Prevention Practitioner
A fantastic opportunity has arisen for a Lipid Management and Secondary Prevention Practitioner (band 8a), 0.67WTE to contribute towards an existing successful secondary prevention service for the patients of Gateshead. This is an innovative service commenced in September 2023 that is pivotal to supporting the national agenda of cardiovascular disease prevention and has been presented at national conferences. The Secondary Prevention Service was also a finalist at the prestigious Bright Ideas in Healthcare Awards 2024.
Main duties of the job
We are looking for a motivated, enthusiastic and highly skilled Lipid Management and Secondary Prevention Practitioner with a prescribing qualification and a keen interest in working in lipid management and secondary prevention of atherosclerotic cardiovascular disease (ASCVD). Under guidance and mentorship of the consultants leading the service, the post holder will provide specialist clinical input and contribute towards existing longitudinal data collection on patient benefit and outcomes.

Job responsibilities
The successful applicant will work closely as part of the MDT (alongside colleagues from cardiology, endocrinology, renal medicine and stroke teams) to triage patient referrals into the service. They will then deliver clinical care in patient clinics, initiating and monitoring patients on guideline-directed medical therapies to achieve target lipid reductions to reduce risk of future cardiovascular events. They will review and promote medicines optimisation for patients who have associated cardiovascular risk factors including diabetes, thyroid disease, chronic kidney disease and hypertension including titration of secondary prevention medication post myocardial infarction.
They will share progress of the project with consultant clinical leads for the service, service line manager for medicine and the key stakeholders of cardiology, endocrinology, renal medicine and stroke teams. There is also the opportunity to be involved in an existing regional lipid specialist advisory group to share ideas and experiences.

Person Specification
Qualifications & Training
* Degree in Pharmacy, Nursing, AHP or equivalent.
* Registered as a Pharmacist with General Pharmaceutical Council (GPhC), Nursing (NMC) or AHP equivalent.
* Advanced Clinical practitioner qualification
Experience
* Experience of leadership
* Experience with patient consultations
* Involvement in projects/audit and ability to use data collection to demonstrate outcomes
* Previous clinical experience with lipid management/secondary prevention/diabetes care
* Previous experience of leading project development and management
* Previous hospital experience
Knowledge
* Evidence of self-reflection and continuous professional development
* Acute Illness Management or similar
* Evidence of innovative practice
Skills and Abilities
* IT literate
* Excellent written and verbal communication skills
* Advanced Clinical Skills or desire to work towards
