Chester South Primary Care Network (PCN) are looking fora part‑time Pharmacy Technician (20 hours a week) to be an integral part of the PCN Pharmacy Team, working within their professional boundaries across four practices within the Primary Care Network.

They will work under the supervision of the Practice Prescribing Leads and the Lead Clinical Pharmacist to ensure the safe, accurate and timely supply of prescribed medication to patients.

They will provide clinical, technical and administrative support to the GPs, Clinical Pharmacists and Medicines Team.

The Pharmacy Technician will support the PCN and practice clinical teams with queries around medication from patients.

The Pharmacy Technician will complete audits around patient safety in the prescribing of medication and present the information for sharing with the relevant clinical teams.

They will also help with the completion of clinical and cost saving projects as directed by Cheshire & Merseyside Integrated Care Board (ICB).


Main duties of the job

To aid in the completion of prescribing scheme projects as directed by Cheshire & Merseyside ICB for cost saving and patient safety purposes.

To provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness.

To assist in the appropriate monitoring and management of prescribing the budgets and to liaise with the medicines management team at the ICB.

To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice protocols.

To work as part of a multi‑disciplinary team whilst in asupporting function to the clinical pharmacists and other clinicians and within agreed prescribing standard operating procedures.

To support the pharmacy team as the PCN point of contact for all medicines related matters.

To encourage cost‑effective prescribing throughout the PCN.

To update and maintain accurate and timely patient medication records on each practice’s clinical system, including advice given and action taken.

To offer patient facing clinics in line with practice needs e.g. offering long‑term condition reviews for conditions such as Asthma and COPD to support practice QoF targets.


About us

Chester South Primary Care Network (CSPCN) is formed of four General Practices: City Walls Medical Centre, Lache Health Centre, Handbridge Medical Centre and Western Avenue Medical Centre. The four practices have been working collaboratively since 2015 and formed CSPCN in 2019 when the Network Contract DES was launched by NHS England.

CSPCN provides a range of services across the GP practices that build on existing GP services and enable greater provision of proactive, personalised, coordinated and integrated health services for 36,000 patients. The services are funded by the Additional Roles Reimbursement Scheme (ARRS) and include Social Prescribing Link Workers, First Contact Physiotherapists, Clinical Pharmacists and Pharmacy Technicians, Care Coordinators, Adult Dieticians and Paediatric Dieticians, Mental Health Practitioners and Adult Mental Health Link Workers.

The ARRS roles are well established and well integrated into the GP practices in CSPCN supported by the PCN Management Team. Our ARRS team consists of dedicated individuals, passionate about their role and united in a wish to provide the best service they can. CSPCN staff are team players who understand the importance of communication and teamwork when working across the practices and within multiple different teams.

The following are the core responsibilities of the PCN Pharmacy Technician. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

To work as part of a multi‑disciplinary team whilst in a supporting function to the clinical pharmacists and other clinicians and within agreed prescribing standard operating procedures.

To support the clinical pharmacists as the PCN point of contact for all medicines related matters.

To process repeat medication requests, including the conversion of acute requests to repeat medicines, changing quantities of medicines.

To issue prescriptions in line with agreed clinical protocols and within your competency.

To encourage cost‑effective prescribing throughout the PCN.

To update and maintain accurate and timely patient medication records on each practice’s clinical system, including advice given and action taken.

To liaise with secondary and primary care colleagues to ensure correct medicines are continued following the transfer of care.

To monitor the practice’s adherence to the prescribing policy.

To support the clinical pharmacists in improving the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards, and clinical and prescribing guidance.

To identify and be proactive in your training needs and development and as agreed with the Clinical Pharmacists.

To support in the delivery of enhanced services and other service requirements on behalf of the PCN.

To participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near‑miss events.

To actively signpost patients to the correct healthcare professional.

To undertake all mandatory training and induction programmes.

To contribute to and embrace the spectrum of clinical governance.

To attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.

To contribute to public health campaigns (e.g. flu clinics) through advice or direct care.


Person Specification


Experience

* Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face.
* Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports.
* Good personal organisational skills.
* Effective time management.
* Demonstrate personal accountability, emotional resilience and work well under pressure.
* Ability to follow legal, ethical, professional and organisational policies/procedures and codes of conduct.
* Ability to use own initiative, discretion and sensitivity.
* Able to work as part of multiple teams; PCN Pharmacy team and the GP practice teams.

* Ability to identify risk and assess/manage risk when working with individuals.
* Sensitive and empathetic in distressing situations.
* Able to finish work tasks.
* Problem solving and analytical skills.
* Ability to maintain confidentiality.
* Knowledge of and ability to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
* Access to own transport and ability to travel across the locality on a regular basis, including to visit people in their own home.
* Willingness to complete the CPPE course (if not already done so), PCN funded.
* Experience of working in primary care, although training can be provided.
* Experienced in using EMIS Web for patient record management.
* Competent in Microsoft Excel for maintaining spreadsheets and managing drug monitoring.


Qualifications

* GCSE grade A to C in English and Maths.
* NVQ Level 3 in Pharmaceutical Science.
* BTEC Level 3 in Pharmaceutical Science, or equivalent qualification with underpinning knowledge.
* Registration with the General Pharmaceutical Council as a Pharmacy Technician.
* It is a requirement of the role to complete the Pharmacy Technician CPPE Course. If the qualification is not already held then candidates must be prepared to undertake the course and complete it.
