We are a small Specialist Nursing Team that provides training and support for patients that choose to dialyse at home. We work closely together with various members of the MDT, GPs, etc., and have close therapeutic relationships with our patient group.
The post holder is an essential part of the unit’s team, helping to streamline the work of the speciality and provide clerical support to the medical and nursing staff within the Home Therapies Unit.
To manage the bookings for all clinics.
Administer new patient referrals.
Organise hospital transport for patients.
Receive and process invoices for patient pharmaceutical and ancillary supplies.
Requesting patient blood tests using ICE and preparing forms and labels
Part-time : 22.75 hours per week

* Deal efficiently and effectively with direct and indirect (i.e. telephone, face to face) enquiries from patients, all levels of medical and nursing staff, GP’s, Social services and other health professionals and staff.
* To communicate appropriately with patients, relatives and visitors who may be fearful, aggressive or have sensitive concerns, needing reassurance. Also, to treat those patients, relatives and visitors who may present with difficulties in communication e.g. hearing impaired, mental health issue, elderly confused patients, in a friendly and welcoming manner whilst maintaining confidentiality.
* To be responsible for ensuring the correct and timely input of patient information onto Patient Administration System (PAS), observing the rules of real time data entry. All aspects of this role should be carried out in line with the Trusts policies and procedures on date collection and confidentiality.
* Attend to all communications, for example emails, phone calls and investigation results on a daily basis, prioritising and taking action as required.
* Receive patients to clinic, confirm demographic details on arrival and ensure regulations for overseas visitors are met.
* Produce new front sheets and labels if there are any amendments.
* Reconcile the clinic to include DNA outcomes to meet with Trust Policy.
